How do you collect data to show improvements in the outcomes due to the learning and performance improvement interventions that you designed and delivered? You can use various data collection methods such as self-administered surveys, one-on-one or group interviews, observations, knowledge/performance tests, and extant data reviews. Among them, I bet you have used survey questionnaires quite a bit.
