Abstract

The magnitude of COPD has begun to overwhelm our healthcare systems both in treatment and associated spending. In Idaho, the death rates related to COPD patients are higher than the national average; therefore, this disease will continue to progress if it is not addressed soon. In efforts to better address this population, we are proposing to implement a designated COPD care team in the hospital systems in order to better manage these patients and diminish readmission rates for COPD related concerns.
