Triangulating Data to Better Understand Barriers to Success in Networks of Diabetes Self-Management Education (DSME) Programs

Abstract

Studies show that individuals with Type II diabetes who enroll in diabetes self-management education (DSME), are better at managing their blood-glucose, have improved eating/dietary habits, and enhanced glycemic control (Lirussi, 2010; Lorig & Holman, 2003). The CDC promotes the creation and maintenance of DSME programs as one of the primary strategies in its 1305 Chronic Disease Partnership grant. Given this focus on disseminating DSME, evaluations of DSME programs must focus not just on individual level health outcomes (e.g. glycemic control), but also on indicators of population access (e.g. number and geographic distribution of programs), as well as barriers and facilitators to the creation/maintenance of DSME programs. Additionally, because individuals may face many barriers in terms of accessing, as well as, completing DSME curricula process measures about the functioning of DSME programs must also be examined to facilitate an understanding of how networks of DSME programs may be improved. The proposed presentation will provide public health practitioners with a three-pronged methodology for evaluating the overall functioning of networks of DSME providers. The first prong involves the ongoing review of statewide data from sources including Medicaid, and BRFSS. The second involves implementing an annual survey of DSME programs, and the third involves a survey or interviews of the clients utilizing DSME programs. Results from the state of Iowa’s evaluations of its network of DSME programs under the 1305 grant program (2013-2018) will be provided and lessons learned regarding maximizing recruitment efforts for surveys and interviews as well as data quality issues will be shared.
